Information we collect
When you use our forms we collect the details you enter: your name, email address, and, only if you choose an SMS category, your mobile phone number. For SMS consent we also record the categories selected, a timestamp, the page where you subscribed, the disclosure version shown, your browser user agent, and your IP address when available.
